That bottom item is a set of the Tshirt, all 10 pics from photosets A and B, along with the photo shown. Basically it's like if you were to buy those three items together (3,300yen(Tshirt)+1,500yen(set A)+1,500yen(set B)), you'd be getting that last photo for free.
